Death of seafarer due to fall from crane cabin
On a vessel at anchor awaiting berthing instructions, two crew members were tasked with cleaning the interiors of the cargo crane cabins, which required maintenance after recent cargo operations. About 30 minutes into the work, one of the seafarers fell 12m from the cabin of one of the cargo cranes to the deck below. The seafarer was fatally injured by the fall.
There was no evidence that fatigue, substance use, or adverse weather played a role in the incident.

Unauthorised boarding and theft from vessel at anchor
An unknown individual boarded a vessel at anchor during night hours. The intruder most likely accessed the vessel from the forward mooring area using a small boat, and entered the accommodation through an unsecured access door. Personal belongings were stolen from crew cabins. CCTV records later captured one individual on the forecastle deck using a heaving line to descend from the vessel. No crew members were injured and there was no operational impact to the vessel.

UK HSE: electrician seriously injured on onshore wind farm
A worker was sent to carry out maintenance work in an electrical substation at an onshore wind farm. There was an electrical flashover, and the worker suffered injuries resulting in life-changing injuries that required multiple surgeries.

BSEE: Exterior Walkway separates from temporary living quarters, putting workers at risk
Three workers were standing on an external third-floor walkway outside of their room located on the platform’s temporary living quarters when the walkway detached from the structure. Two of the workers were able to open the door and returned inside their room. The third worker moved away from the detached section while remaining on the walkway. After the crew confirmed all workers were safe, personnel secured the stairway and walkway with chain bindings to prevent dropped objects and further movement.

LTI – crew member squeezed between buoy and cargo rail
During anchor handling operations, a crew member was crushed between a large buoy and a cargo rail. The crew member was freed and had to be med-evaced. The incident occurred when the deck crew was in the process of connecting two 5-ton sub-surface buoys to a chain adapter as part of a multi-step hookup sequence. After securing an 800m fibre rope in the shark jaw, the team began preparing the buoys for connection. The first buoy had already been positioned using the starboard crane. The second buoy was then dragged toward the connection point using the port crane, while the buoys remained partly interconnected through their stock eyes.
