More positive stories

During welding activities, good adherence was observed of respiratory PPE. The welder was seen to be properly equipped with helmet and respiratory protection, demonstrating strong safety awareness. Overall, great safe practice was observed; minor attention to hose arrangement was recommended to reduce trip hazards.

BSEE: Prevent fires by inspecting cords, plugs and welding leads before use

The United States Bureau of Safety and Environmental Enforcement (BSEE) has published Safety Alert 515, relating to recent fire incidents involving damaged extension cords, plugs and welding leads.

Lamp fixture fire in office cabin

The alarm was detected on the Fire Alarm Panel at 21:59. Personnel went to the cabin and upon opening the door saw fire and smoke (from the ceiling light). A fire extinguisher was used, extinguishing the fire at 22:01. There were no injuries.

Floodlamp causes scaffold board to start smouldering

A closer inspection revealed that one of the scaffold boards was smouldering beneath an inverted floodlight. Relevant personnel were immediately informed. The floodlight was found to be positioned upside down and was subsequently isolated. The affected scaffold board was cooled using a wet rag, and the board was later removed and replaced.

Arm injury – need for focus on safe isolation and task control

The compactor unexpectedly activated while the person was manually testing a proximity sensor through an opening at the top of the unit, resulting in their arm being trapped between the ram and the top cover. The worker ended up on restricted duty for a day due to bruising/stiffness.
