Near miss: worker suffers electric shock
The incident occurred whilst the worker was performing a small oil clean up on a 50t crane pedestal. The worker suffered the electrical shock when he brushed his hand against one of two disconnected cables coiled on nearby cable trays.
The worker was subsequently visited by the vessel medic and was unharmed.

Unsafe use of electrical equipment in cabins
During an inspection of a members’ vessel, crew members were observed inserting 2-pin electrical chargers directly into 3-pin vessel sockets to power their personal equipment in the accommodation. It appeared that there were no appropriate adaptors available onboard.
Inserting 2-pin plugs into 3-pin sockets can physically damage the internal components of the socket. Over time, this may cause loose connections, which are a well-known cause of electrical arcing, overheating, and even fire.

UK HSE: load falls from lorry and kills cyclist
A metal heat exchanger, weighing over 2.5 tons, fell from a lorry and killed a passing cyclist. The UK Health and Safety Executive fined the road haulage company responsible. See press release.
A heavy lorry carrying four heat exchangers collectively weighing over 10,000kg, went round a bend, when the load shifted in the vehicle causing one of the straps securing the load to snap. This resulted in one of the heat exchangers falling from the lorry, killing a cyclist who was cycling alongside the vehicle.

High potential incident: Worker injured when opening a flanged assembly
The team had, successfully and without any problems, already dismantled three valves / ESDV as per the plans for that night. As the last valve was being disconnected, the Grayloc connection between the jumper and valve, along with the gasket inside the line, were ejected with force. The ejected gasket hit one worker on the forehead, resulting in a bleeding wound. The injured person remained conscious throughout and was assisted to the medic for treatment. The worker was treated by the medic and received seventeen stitches to the wound.

LTI: Hand injury during capstan maintenance
A crew member was injured when their hand was trapped between a wire clamp on the underside of the capstan (where the greasing point was located) and the deck. The incident occurred during capstan maintenance, when the crew member and an officer were greasing the capstan. The crew member signalled the officer to engage the capstan, the unit started turning, and the crew member got his hand trapped, causing a serious injury which required medical assistance. The crew member was subsequently taken ashore to hospital for further treatment.
