Diver entanglement – uncontrolled equipment in the water column
A diver started work welding an anode to a piggy-back riser clamp at 1.5msw. When starting the job, the diver hung a rubber mat with welding rods connected with a rope and carabiner to the rope suspending the welding cables. It resulted in a compromised work area. Due to the swell at 1.5m depth the connected light weight items were floating uncontrolled in the water column.

Diver entanglement – umbilical caught around an anode
The diver was unable to release his umbilical during regular dive time. As the umbilical could be slackened the diver started with the in-water decompression stops. The stand-by diver was launched and was able to free the diver’s umbilical.

Unsecured sheave pin fell from crane
It fell from a 600 tonne crawler crane supplied by a third party. The pin likely fell from 30m height after the crane wire came into contact with its split pin, causing it to snap or to become dislodged. No-one was in the line of fire at the time, and there were no injuries. However, the incident posed a significant potential risk.

Heaving line snap-back causes injury
The sudden release of energy caused several workers to lose balance and fall onto the quay surface. One worker sustained a head impact when falling because their helmet chin strap was not properly secured, allowing the helmet to shift and fail to protect their head.
The injured person was taken to hospital, treated, and released after a short period, and the incident was classified as a Medical Treatment Case

Failure of moonpool railing system caused man overboard situation in moonpool
A crew member was working in the moonpool area when they lost balance and leaned against a removable railing section intended to act as a primary safety barrier. The railing post failed, causing the gate to swing inward toward the moonpool. They fell into the moonpool but managed to grab hold of the guidewire, preventing an uncontrolled fall into the sea. They gradually slid down the guidewire until reaching the guidewire weight, at water level inside the moonpool.
Co-workers intervened immediately, raised a man overboard alarm, and using a boathook pulled the crew member to safety. The crew member was recovered within 52 seconds of the initial fall. The crew member sustained a dislocated shoulder and was transported to hospital for treatment.
