BSEE: Failure to identify and restrict access to faulty gratings
During a facility walkthrough, operator personnel had identified grating near a skid as a hazard due to corrosion and installed hard barricades to prevent access. However, shortly thereafter, operator personnel installed two scaffolding boards on top of the previously identified grating hazard and removed the hard barricades so work could proceed

Hand injuries dealing with sharp objects
A member reports two separate incidents relating to sharp objects.
Incident 1: A deck crew member sustained a finger injury while cutting duct tape connecting two halyards during maintenance. The cutter blade sliced through the glove, resulting in a cut that required medical attention and stitches.
Incident 2: A crew member sustained a severe hand injury while manually adjusting a package with a damaged metal rim on a pallet. The sharp rim caused a deep cut near the thumb, requiring 12 stitches to stop the bleeding.

MAIB: LTI – fingers lost during mooring operations
A crew member was badly injured when their fingers were crushed during mooring operations. A port tender was transferring two workers to a barge moored on a busy river. Two boats were already tied up to the barge so the skipper placed the tender alongside one of these. A crew member ran a mooring rope between a cleat on the port shoulder of the tender to one on the boat secured to the barge, holding the tender in place while the two workers boarded the barge. The tender had a much lower freeboard than the boat it was tied to, and the mooring rope was at a steep angle.

Injury to thumb during lifting operations
The incident occurred during vessel demobilization alongside. During the lift the truck driver encountered difficulties and asked for help. In response, the crew member stepped onto the trailer, positioning themselves at one end while the truck driver stood at the other. At the time of the incident, the crane was holding the load approximately 30cm above the trailer. A verbal command was given to lower the load, but the crew member was still repositioning a wooden block for better placement when the load came down trapping his thumb. He managed to pull his hand out from under the load, sustaining an injury. First aid was administered and crew member was then taken to hospital.

LTI: Back injury in Confined Space (Tank Entry)
The incident occurred during completion of planned maintenance. Whilst attempting to get out of the confined space through a lightening hole by moving feet-first (face up), the worker’s hands slipped and they fell back, striking their lower back on a beam at deck level. The worker was still able to exit the compartment and confined space unassisted. Onshore medical assessment/treatment, however, was subsequently required.
