MSF: hose got tangled in an azimuth thruster
On a vessel transiting to port during rough weather a fire cabinet on the main deck opened, resulting in the fire hose being washed overboard. The fire hose subsequently became entangled in the vessel’s starboard azimuth propeller.

Mooring rope caught in stern thruster
A vessel was preparing to depart with a pilot onboard. Local traffic control instructed the Pilot that the vessel must remain alongside until another vessel departed and passed astern, and that the vessel was not permitted to use thrusters until the passing vessel was clear.

Unauthorised firing of rocket line
A vessel was preparing for the connection of a tow wire for pulling back a tanker. The vessel was manoeuvring astern, approx. 10m from the tanker stern to receive heaving line and messenger line. The tanker was stopped on DP, bow to current and weather.

Shoulder injury during intrusive work on pressurised hydraulic system
