MSF: Unsecured bulk hose near miss
During hose transfer operations on a platform supply vessel in challenging weather, a hose was left unsecured on deck. The hose subsequently slipped off the deck, nearly striking a crew member. The operation was promptly aborted by the Captain, and a debrief was held to discuss the incident.

Unsafe handling of gas cylinders
An AHTS vessel was alongside for maintenance activities being carried out by a third-party contractor. The vessel’s Chief Mate conducted a pre-job verification for planned hot work activities (gas cutting and welding), and noticed unsafe positioning of compressed gas cylinders on the quayside. The gas cylinders had been laid on the ground by the third-party contractor, without any racks or securing arrangements (one was stored vertically/unsecured), creating an unstable condition and increasing the risk of movement, impact, or damage.

Working in a confined space without a Permit to Work
After consulting with the duty engineer it was confirmed that there was no confined space entry permit in place and the work was stopped. Our member regarded the situation as a near miss as the correct safety controls were missing.

Failure of A-frame fold-down platform
The incident occurred while the vessel was alongside for mobilisation. During load checks of the sheave block, a crew member stepped out onto the aft side A-frame platform, which had a hinged centre platform. As they did so, the hinged centre platform gave way and fell to the sheltered deck (approximately 12 metres below). The crew member was left suspended by their safety harness. Operations were immediately stopped, and the person was quickly recovered and sent to hospital for a complete medical examination. They suffered minor bruising and abrasion.

Battery power bank explodes in cabin
The crew member to whom it belonged was woken up by the explosion and started to extinguish the fire with a soaked towel. A “pre-warning” fire alarm sounded on the bridge and crew proceeded to the cabin to investigate the alarm, but the fire was already put out. There was minor damage to fittings and furniture in the cabin.
This had the potential for escalation to a larger cabin fire, smoke damage to adjacent areas, activation of fire systems, loss of accommodation, and possible injury from burns or smoke inhalation. The fire was extinguished. The crew member reacted quickly and smothered the fire with a wet towel.
