On a more positive note…
An excellent example was seen of a toolbox talk (TBT) being done very well. The TBT occurred on the bridge just before the vessel entered the 500m zone. The Chief Officer led the talk, presented the STOP card, and reinforced the company policy for Stop Work Authority. There was noticeable engagement (rather than bored-looking team members) from crew, which reflected a strong safety culture.

Injury after fall from vertical ladder
Two crew members were performing routine engine room fire watch and thruster space rounds checking oil pressure and temperature checks, when one of them was injured falling off a vertical ladder. he incident happened when they were climbing up the 3m high ladder; one of them was holding a fire patrol device in one hand. That crew member lost grip on the handrail at the top of the ladder, fell and hit their head on the edge of a steel bund on the deck, sustaining a minor cut to the right eyebrow.

MSF: Burn to arm from contact with tumble dryer
The Marine Safety Forum (MSF) has published Safety Alert 25-13 relating to a crew member burning themselves on a tumble dryer. A crew member suffered a minor burn on the left lower arm when reaching into a tumble dryer, and their arm touched the metal frame around the door. The incident occurred when the crew member opened the tumble dryer mid-cycle, before it had finished the program, and therefore it had not completed the cooling down session.

LTI: serious injury to thumb when pipe fell during maintenance
A 2nd engineer on a vessel suffered a serious injury to the left thumb whilst dismantling a grey water pipe. The 2nd engineer and 3rd engineer began dismantling the vertical section of the grey water pipe in the engine room; they first removed bolts from both flanges (top & bottom) and left two bolts of top flange in loose position to keep the pipe in position. Whilst removing the lower U-clamp, the pipe unexpectedly fell about 1.4m. The weight of the pipe was about 25 kg. The left thumb of the 2nd engineer was caught between the falling pipe and the supporting U-clamp’s plate. This accident resulted in a partial amputation of the thumb.

Japan Transport Safety Board: two confined space fatalities
The Japan Transport Safety Board has published report MA2025-4 into a fatal incident which occurred in May 2024 on a bulk carrier moored at a port in Japan. Two stevedores collapsed inside the cargo hold of the vessel during unloading operations. One died and the other was seriously injured. The incident occurred shortly after they entered the hold without conducting atmospheric testing.
