Two hand injuries caused during mooring
Following the replacement of a damaged mooring line on an FPSO, two crew members installed protection to secure the new line. The task occurred during rough weather. As the mooring line slackened, one of the workers grabbed it to position the protective material. As they did so, the swell caused the vessel to move. The mooring line tightened, trapping and injuring the hand of the worker, who was not wearing suitable gloves at that point.

MSF: Watertight door fatality
An individual at a marine worksite was found in between the sliding hydraulic watertight door and the door frame. He was found unconscious and not breathing. He was brought to sick bay and assessed with no signs of life. Although actual causes have yet to be determined since the investigation is still ongoing, the purpose of sharing this alert is to facilitate reflective discussions with vessel marine crews and interested parties on the dangers posed by watertight doors.

Hand crushed during coiled tubing reel handling
During the final stages of a coiled tubing operation, a completed reel was being lifted from the spooler and lowered into a support cradle. The reel, weighing 2600 kg (without tubing), was being aligned manually by the injured person, who placed their left hand on the reel to assist with positioning.
As the reel was landing, the tube securing frame on the reel contacted the side of the support cradle and shifted. This movement trapped the worker’s hand between the tube securing frame and a reel bar. The lift was immediately halted, and the securing frame was released. The worker was transported to a local hospital, and treated for severe crush injuries, resulting in partial amputations of three fingers of the left hand.

MSF: Grease gun hand injury
The Marine Safety Forum have published Safety Alert 25-08 relating to a hand injury suffered when a grease gun burst. The incident occurred during routine greasing of a tensioner unit using an electrically powered battery grease gun, when the hose burst and grease punctured the operator’s hand. The crew member was wearing full PPE and performing the task in accordance with safety procedures that were in place at the time. As the crew member attempted to remove the greasing hose from the greasing point, the hose burst on his right hand. The high-pressured grease passed through the damaged hose causing a penetration through his glove, leading to a small open wound on his skin. The crew person immediately attempted to remove as much grease as possible from the wound and proceeded directly to the onboard medic for evaluation and treatment. Following consultation it was decided to sail back to port for medical attention at local hospital.

BSEE: Electromagnetic lifting device dropped steel plate
The United States Bureau of Safety and Environmental Enforcement (BSEE) has published Safety Alert 500 relating to a defective lifting device dropping a steel plate during drilling operations, and two persons were injured. The incident occurred while transferring a partial 3/8” (95mm) steel plate sheet using a magnetic lifting device. During the lift, the steel plate unexpectedly detached from the magnet and fell approx. 1.2m, landing on top of both individuals’ left feet. Both personnel reported bruising and swelling and were evacuated for medical evaluation.
